Case Manager (Full-time, Day shift)

What You'll Be Doing Assesses the circumstances of clients who are Hudson County residents and HIV-positive and arranges an array of services to help them maintain health and functionality; educates clients and other members of the community about HIV; participates in required training and agency development efforts; represents agency as directed by supervisor; attends required meetings; collaborates with medical and other service providers. What We're Looking For Education: Bachelor’s degree in social work related field. Experience: 2 + years related experience. Skills: Excellent interpersonal and communication skills. Monday through Friday - 8:00 AM to 4:30 PM
