About the role
The CCBYS Prevention/Intervention Case Manager will provide crisis intervention and prevention services to youth and families, collaborating with law enforcement, schools, and juvenile justice professionals.
Responsibilities
- Provides crisis intervention and prevention services to assess youth/family safety and related needs, develop stabilization plans, and prepare for services.
- Available to law enforcement 24-7 per rotation and to schools, juvenile justice professionals, and DCFS for crisis prevention.
- Performs next-day follow-up, intake, and assessment documentation and support for crisis intervention and prevention referrals.
- Maintains relationships and contact information databases for law enforcement agencies and school/juvenile justice programs.
- Serves as a liaison and provides program information and training to crisis intervention and prevention referral sources.
- Manages and carries out 3- and 6-month client follow-up contacts.
- Gathers and monitors crisis-related data (and other data as needed) using SDS and eCornerstone.
- Writes and delivers accurate reports on a timely basis.
- Assists staff members for CQI Peer Record Review preparation in coordination with office administration assistants.
Requirements
- BS/BA degree
- Valid driver’s license and acceptable driving record with OHU required levels of coverage for auto insurance
- Physical Exam/TB

- Proficient computer skills in the Microsoft Windows environment
- Highly organized, flexible, able to manage multiple priorities/projects, and handle tight deadlines
- Ability to function well in a team environment

Pay
Salary range: $35,000 - $45,000 annually
Schedule
Full-time, 40 hours per week
