About the role

Mount Carmel Guild Behavioral Health System is a leading provider of mental health and addiction services in Northern New Jersey. We are seeking a Full Time Case Manager for our Partial Care department in Jersey City.

Responsibilities

- Provides case management and support to Partial Care clients. - Facilitates groups. - Documents according to regulatory standards. - Carries a caseload. - Interfaces with Partial Care psychiatrist. - Maintains collateral contacts with family members and providers in the community on behalf of clients.
