Job Responsibilities
- Maintain firm adherence to the resident's "Bill of Rights," as they relate to the facility
- Communicate with the resident's care providers
- Assessments and coordination of admission/re-admission from the community, nursing homes and hospitals
- Care coordination and supervision for residents
- Maintain clear and concise documentation in accordance with facility requirements
- Manage financial case management duties, including Medicaid applications, recertifications, and benefits coordination
- Track and follow up on pending financial cases to ensure timely resolution
- Coordinate with families, financial institutions, and government agencies regarding resident benefits and entitlements
- Monitor resident accounts and liaise with billing/business office on financial status changes
Qualifications/Requirements
- Prior experience in Long Term Care is a PLUS or 1 year of experience as a Case Manager required
- Experience with Medicaid applications and financial case management strongly preferred
- Previous ALIS experience is a plus
- Excellent phone skills and ability to work with families/other care providers
- Strong organizational skills, passion and compassion for the population
- Able to work cooperatively with multidisciplinary team
- Comfortable managing both clinical case management and financial/benefits coordination responsibilities
