About the role

The Northeast Florida Healthy Start Coalition is seeking a full-time Case Manager to join the Fatherhood PRIDE division. The ideal candidate will be responsible for conducting community outreach, enrolling fathers into the program, providing case management services, facilitating fatherhood workshops, and collecting evaluation data.

Responsibilities

  • Facilitate fatherhood workshops including, but not limited to 24/7 Dad, Inside Out Dad, Boot Camp for New Dads, Mom’s Gateway, Growing Responsible Fathers, etc..
  • Carefully coordinate workshop schedule, location(s), guest presenters, instructional materials, and supplies.
  • Provide onsite support to outside facilitators that provide financial literacy and workforce development.
  • Ensure fathers complete intake process to enroll into program.
  • Provide holistic and comprehensive case management services to all fathers including: intake and benefit assessments, goals setting, long-term case-plan development, weekly case-plan development, progress monitoring, individual money management, tenant education, advocacy, and referrals.
  • Engage regularly with the fathers in the program to ensure that members complete program requirements, which include: parenting workshops, financial literacy workshops, workforce development services, and case management services.
  • Maintain case-load to meet enrollment and completion numbers reported at team meetings.
  • Monitor progress of participants on a regular basis and document all interactions.
  • Connect fathers with needed and available community resources.
  • Follow-up with fathers and agencies as appropriate to document use/success of referrals.
  • Input accurate and complete data for all contacts with members into agency database.
  • Ensure that all documents submitted on behalf of a father are valid.
  • Provide all required documentation in a timely matter, which may include father follow-up outcome evaluation, client contact sheets, and evaluations.
  • Evaluate/data entry - Follow data collection protocols for reporting data; input accurate data within prescribed timeframes.
  • Plan, coordinate and implement quarterly gatherings for fathers in program.
  • Plan, coordinate implement and attend special events like the Father’s Day Barbecue competition, Barbers for Babies, Infant Mortality Awareness events, etc..
  • Attend all staff and team in-services, conferences, workshops and community meetings.
  • Participate in grantee initiatives.
  • Conduct outreach to recruit fathers to enroll into program at health fairs, community events, the court system, program special events, and through community partners.
  • Assist program director and program supervisor identify, locate, and engage partners to refer fathers.
  • Network within the community to provide case management resources for fathers.
  • Maintain relationships with current partners through regular check-ins to see if they need referral forms, brochures, or if they have questions about the program.
  • Perform other duties as assigned by the Director, Fatherhood Project.

Requirements

  • Bachelor’s Degree in social services or related field or equivalent work experience in the field.
  • Ability to communicate both orally and in writing with individuals for a wide variety of socio-economic backgrounds including professionals, non-professionals, and the general public, concerning a wide range of subjects and /or situations.
  • Excellent computer proficiency (MS Office Word, Excel and Outlook).
  • Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service.
  • Knowledge of the criminal justice system and child support court systems.
  • Ability to work flexible traditional and non-traditional hours.
  • Ability to establish and maintain effective working relationships with Coalition partner agency staff and the general public.
  • Ability to conduct discussions, interviews, and meetings.
  • Maintain professional boundaries.
  • Experience in making public presentations and public speaking.
  • Ability to maintain confidentiality.
  • Experience working with diverse communities, organizations and agencies preferred.
  • Knowledge of counseling principles and record-keeping techniques.
  • Knowledge of community resources.
  • Experience working with persons in crisis.
  • Ability to work independently and to carry out assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.
  • Requires reliable transportation as travel is required, valid driver’s license, good driving record, and automobile insurance.

Physical Requirements

  • Ability to safely and successfully perform the essential job functions consistent with the ADA, and other federal, state, and local standards.
  • Must be able to travel to alternate locations.
  • Regularly required to sit, stand, walk, speak, and hear while performing the duties of the job.
  • The position requires extensive computer use so the employee must have sufficient hand dexterity to use a computer keyboard and be capable of reading a computer screen.
  • Lifting/carrying up to 25 pounds.
