About the role
We're hiring a Case Manager (CDCA) specializing in Substance Use Disorder (SUD) and Mental Health services in Toledo. The position offers a competitive salary range of $21-$24 per hour, based on experience.
Responsibilities
- Meet with clients upon admission to assess needs and begin case management planning.
- Aid in documenting for FMLA, short-term disability, legal issues, and more.
- Conduct weekly one-on-one case management sessions.
- Collaborate with patients and families to ensure open communication and effective aftercare planning.
- Coordinate discharge logistics and medical records with receiving facilities.
- Facilitate engaging group sessions focused on SUD and mental health.
- Maintain program compliance, ensure confidentiality, and keep the vibe professional but caring.
Requirements
- A bachelor’s degree or equivalent experience in behavioral health.
- At least 2 years of experience in the field of behavioral health or substance use recovery.
- Bachelor-level SUD certification (LADC, CRSW, CADC, or equivalent).
- Valid driver’s license.
