Case Manager | Care Coordinator| PRN | Rotating Weekends

Responsibilities

• Participates in the development of the patient's plan of care
• Facilitates efficient throughput
• Conducts utilization review
• Promotes appropriate utilization of resources
• Identifies needs and establishes safe and appropriate discharge plans

Qualifications

• Required Education: Associate Degree in Nursing, -OR- Masters in Social Work or related field
• Necessary Skills: Excellent verbal and written communication skills, excellent organizational skills, strong personal computer skills (Email, Word, Excel), good typing skills (20 wpm)
• Required Licensure/Certifications: Licensed to practice nursing in the state of Florida for those meeting ASN/BSN education requirement - Masters in Social work (or related field) accepted in lieu of RN licensure
• Required Experience: 1-2 years - Discharge planning, case management, managed care, or Registered Nurse experience in a medical setting
