About the role

This position provides comprehensive care coordination for patients, ensuring situational and appropriate care based on patient need and payer requirements. You will be accountable for the clinical quality of care coordination services, identifying and resolving barriers to effective patient care, and empowering patients and families to participate in the discharge planning process. The role involves planning for safe discharge, continuity of care, and recognizing the unique needs of diverse patient populations.

Responsibilities

  • Manage individual patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
  • Lead process improvement activities for populations of patients to achieve optimal outcomes.
  • Collaboratively develop and manage the interdisciplinary patient discharge plan and communicate it effectively across the continuum of care.
  • Maintain knowledge of Medicare, Medicaid, and other program benefits to assist patients with discharge planning and choices.
  • Understand community resources relevant to health care, end-of-life dynamics, substance abuse, abuse, neglect, and domestic violence.
  • Establish and promote collaborative relationships with physicians, payers, and other members of the health care team.
  • Collect and communicate pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.
  • Educate internal members of the health care team on case management and managed care concepts.
  • Facilitate integration of case management concepts into daily practice.
  • May supervise other staff and confer with supervisors on unusual situations.

Requirements

  • RN: Must possess knowledge of case management or utilization review, typically obtained through a bachelor's degree in case management or health care. Requires current Registered Nurse (R.N.) license in the state worked.
  • Social Worker: Requires a Master's Degree in Social Work. Must be a Licensed Master Social Worker (LMSW) (or equivalent) or Licensed Clinical Social Worker (LCSW), or have an MSW with the requirement to become licensed within 6 months of hire. For assignments in an acute care setting, Basic Life Support (BLS) certification is required.
  • Proficiency level typically achieved with 2 years of clinical experience.
  • Must demonstrate critical thinking skills, problem-solving abilities, effective communication skills, and time management skills.
  • Must demonstrate ability to work effectively in an interdisciplinary team format.
  • May require rotating call based on the acute facility need.
  • For acute care Case Management positions, Banner Registry and Travel positions require a minimum of one year of Case Manager experience in an acute care hospital.

Preferred Qualifications

  • Certification for CCM (Certified Case Manager) preferred.
  • Additional related education and/or experience preferred.

Schedule

  • Full-time, 40 hours per week.
  • 4 ten-hour shifts (Monday-Friday, 7:00 AM - 5:30 PM).
  • Every 3 weeks includes a weekend rotation (Saturday & Sunday).
  • Holiday rotations are required in this role.

Pay

Estimated pay range: $35.37 - $58.95 per hour. Banner Health is committed to pay equity and transparency. Actual pay is determined at offer based on years of relevant work experience, education, certifications, skills, geographic location, and a review of current employees in similar roles.
