Case Manager - Behavioral Health - Full Time

About the role

Elliot Behavioral Health Services is staffed by a team of mental health professionals who tailor both evaluation and treatment services to each individual patient. Psychotherapy services are available independent of or in conjunction with other treatment modalities, to provide an integral treatment approach to most problems. Medication evaluation and management services are designed to treat each individual patient's potential need and ability to benefit from medication intervention in biologically based conditions that provide an effective, yet less intrusive treatment approach. Communication of results and consultation with family, parents, schools and medical or mental health professionals, ensures that specific treatment approaches are implemented to meet each patient's need. Behavioral Health Case Manager provides clinical social work interventions with patients (pediatrics, adolescents, adults, and geriatrics) and families that include assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress.
