About the Role

Behavioral Health Case Manager provides clinical social work interventions with patients (pediatrics, adolescents, adults, and geriatrics) and families. This includes assessing treatment needs, developing, monitoring, and evaluating treatment plans and progress. The role also provides services that empower clients to meet goals as independently as possible, addressing personal and environmental issues to maximize emotional, social, and physical wellbeing and the appropriate and efficient use of health care services across the continuum.

Promotes efficient coordination of patient care through collaborative practice with the health care team, ensuring continuity of care and systematic evaluation of patient care. Coordinates patient care through a continuum of care model to achieve optimal quality outcomes in relation to clinical care and cost effectiveness.

The Behavioral Health Case Manager role is primarily remote, and the employee must utilize all designated communication tools to collaborate with the EBHS team. Home office supplies will not be provided by Elliot Health System.

Responsibilities

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-focused.
  • Provide on-going support and case management expertise to clients' needs.
  • Educate and support transition of patients to community-based services.
  • Ensure that all client case management plans are current and address identified needs.
  • Communicate effectively with a variety of individuals representing diverse cultures and backgrounds, demonstrating sensitivity, tact, and diplomacy.
  • Continually monitor and evaluate each client’s progression through their case management plan, and develop corrective action revisions as needed.
  • Maintain accurate documentation of service objectives and outcomes, as well as other services in accordance with Federal, State, and Solution Health guidelines.
  • Provide information, referrals, and facilitate linkages for clients to mental health treatment, temporary and permanent housing, and public assistance programs.
  • Actively participate in client-related meetings and trainings.
  • Administer screenings for patients for mental health and substance abuse disorders.
  • Maintain client-related data systems, including case notes and complete EPIC entries.
  • Prepare case management-related reports, including outcomes and successes.
  • Adhere to strict boundaries and professional ethics in the care of others.
  • Assess crisis situations and respond/refer appropriately in a timely manner.

Requirements

  • Master’s degree in social work, Mental Health Counseling, or related field.
  • Experience in a social service setting with working knowledge of case management systems and planning techniques.

Monday, Wednesday, Friday: 8:00 AM – 4:30 PM (in office)

Tuesday, Thursday: 8:00 AM – 4:30 PM (remote)
