Responsibilities

  • Assess treatment needs; monitor and evaluate treatment plans and progress; monitor staff performance.
  • Determine clients’ requirements by completing intake interviews; evaluate need for therapeutic medical, psycho-social, and psychiatric evaluations; review therapist evaluations, treatment objectives, and plans.
  • Schedule clients with an Addiction Psychiatrist if needed.
  • Conduct monthly scheduled appointments with clients according to the appointment schedule.
  • Establish treatment programs by setting schedules and routines; coordinate services being provided; arrange resources.
  • Monitor cases by verifying clients’ attendance; observe and evaluate treatments and responses; advocate for needed services, obtain additional resources; intervene in crises; provide personal support.
  • Maintain clients’ records by reviewing case notes, logging events and progress.
  • Communicate clients’ progress by conducting monthly coordination meetings and evaluations with the client’s clinician team.
  • Prepare clients’ discharge by reviewing and amplifying discharge plans; coordinate discharge and post-discharge requirements; provide resources.

Requirements

  • Bachelor’s degree in social work, Psychology, or related field; or Associate’s degree with at least 2 years of Case Management experience; or equivalent combination of education and experience.
  • Professional or personal experience supporting individuals with disabilities, mental illnesses, or challenging behaviors highly preferred.
  • Must be over 21 years of age.

Pay

Starting salary of $48,000 annually, with significant potential for higher earnings based on performance and growth opportunities.

Schedule

Full-time hybrid position. Expected to commute to the office in Sandy, Utah at least 2-3 times a week.
