Case Manager - Assertive Community Treatment

About the role

The Case Manager - Assertive Community Treatment is responsible for providing intensive case management services to individuals with severe mental illness who are experiencing homelessness.

Responsibilities

  • Conduct home visits and provide direct support to clients
  • Coordinate services from multiple community agencies
  • Develop and implement individualized treatment plans
  • Provide crisis intervention and stabilization services
  • Maintain accurate client records and documentation

Requirements

  • Bachelor’s degree in Social Work, Psychology, or related field
  • Valid driver’s license and access to reliable transportation
  • Experience working with individuals with severe mental illness
  • Ability to work flexible hours including evenings and weekends

Qualifications

  • Knowledge of community resources and service providers
  • Strong communication and interpersonal skills
  • Proficient in Microsoft Office Suite
