Case Manager – Adult Mental Health & SUD Team (Spanish Bilingual)

About the role

Clackamas County's Health Centers is seeking a Bilingual (English/Spanish) Case Manager to join and support the Adult Mental Health and Substance Use Disorder (SUD) Team. The selected case manager will partner closely with CareOregon’s Strategic Healthcare Investment for Transformation (SHIFT) initiative—a statewide effort to transform specialty behavioral health organizations so that individuals with behavioral health needs remain at the center of care, and care teams can thrive. Through a collaborative approach, SHIFT builds client-driven, outcomes-focused, team-based care models that eliminate health inequities and ensure timely access to care.

The Case Manager performs skills training, case management, care coordination, and medication training for individuals with mental health diagnoses or co-occurring mental health and substance abuse problems within our behavioral health program. The Case Manager also works with family and community partners to provide education and coordinate access to resources, benefits, and support. This position includes in-home visits and transporting individuals as part of a service delivery model.

Case managers provide services to vulnerable individuals, many of whom may have limited resources, and should thrive in both independent and team-based settings.

Responsibilities

• Coordinate comprehensive case management services for adults with behavioral health and/or substance use needs, including assessment, treatment planning, resource coordination, progress monitoring, crisis intervention, safety planning, and discharge planning.
• Collaborate with psychiatric providers, primary care teams, nurses, insurance companies, and community partners to support client stability, continuity of care, and successful transitions between services.
• Conduct client assessments and interviews, gather and evaluate clinical information, develop and implement individualized treatment plans, identify service needs, and coordinate referrals to appropriate internal and external providers.
• Maintain accurate and timely electronic health records, documentation, and case files in compliance with Oregon Administrative Rules (OARs), agency policies, and regulatory standards.
• Provide counseling, skills training, education, and case management services that support clients in achieving treatment goals, improving daily living skills, increasing independence, and enhancing overall well-being.
• Partner with internal teams, healthcare providers, and community organizations to coordinate services, resolve barriers to care, and ensure clients receive appropriate supports and resources.
• Advocate for clients by collaborating with families, service providers, schools, law enforcement, courts, and community agencies; respond to crises and help resolve complex client concerns.

Requirements

• Qualified Mental Health Associate (QMHA) or (QMHA-R) as established by the Mental Health and Addiction Counseling Board of Oregon (MHACBO) at the time of hire.
• Bachelor’s degree in a behavioral sciences field; or a combination of at least three years relevant work, education, training, or experience.
• Fluency in Spanish/English, both oral and written (bilingual skills to be validated/tested at time of interview and/or selection).
• Must possess and maintain a Basic Life Support (BLS) certificate.
• Must pass a post-offer, pre-employment drug test and a criminal history check, which may include national or state fingerprint records.
• Driving is necessary for County business. Must possess a valid driver’s license and maintain an acceptable driving record throughout employment.

Preferred Qualifications

• Experience working in a primary care or behavioral health care setting.
• Experience providing case management services for clients diagnosed with severe and persistent mental illness (SPMI) and/or substance use disorders (SUD).
• Experience working within a multi-disciplinary treatment team.

Pay

Annual Pay Range: $63,032.03 – $79,424.01. Bilingual Spanish/English skills qualify for an additional 5% of base compensation. Salary offers will be made within the posted range based on candidate’s relevant experience.

Schedule

This position works 40 hours during a standard workweek of Monday through Friday. Specific hours will be discussed at the time of offer. The position is eligible for hybrid on-site/telework based on department needs. Telework locations cannot be located outside of Oregon or Washington. Washington residents must perform 50% of their time on-site at Clackamas County.

About the Department

Clackamas County Health Centers provide direct medical, dental, and mental health services to residents through Integrated Primary Health Care Clinics, mental health clinics, and School-Based Health Centers.
