The Providence Center Case Manager ACT Harmony Team will coordinate services for consumers to minimize stressors and reduce psychiatric symptoms.
Duties and Responsibilities
- Coordinate a variety of services
- Provide supportive counseling, problem solving, and limit setting
- Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services
- Monitor client status during spend down process, complete necessary DHS applications
- In cooperation with Managers or Emergency Services Staff, assist with assessments, crisis interventions and risk management as needed
- Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves
- Transport consumers in own vehicle to planned activities and appointments
- Complete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements within required time frames and abide by TPC confidentiality policy
- Maintain cooperative relationships with Center staff, consumers, community agencies and the public
- Attend mandatory in-service trainings and other trainings required
- Attend case presentations and conferences
- Other duties as assigned
Requirements
- Associate's degree required. Bachelor's degree from accredited four-year College or University in Human Services or related field preferred
- Experience in Human Services field preferred
- Bilingual Spanish candidates preferred
- Must have valid driver's license and insured auto
- Must be able to work flexible hours
This position is eligible for a $2500 retention bonus.
