Case Manager ACT Harmony Team $2500 retention bonus

The Providence Center Case Manager ACT Harmony Team will coordinate services for consumers to minimize stressors and reduce psychiatric symptoms.

Duties and Responsibilities

• Coordinate a variety of services
• Provide supportive counseling, problem solving, and limit setting
• Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services
• Monitor client status during spend down process, complete necessary DHS applications
• In cooperation with Managers or Emergency Services Staff, assist with assessments, crisis interventions and risk management as needed
• Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves
• Transport consumers in own vehicle to planned activities and appointments
• Complete recordkeeping in accordance with Health Information Services (HIS) and other TPC requirements within required time frames and abide by TPC confidentiality policy
• Maintain cooperative relationships with Center staff, consumers, community agencies and the public
• Attend mandatory in-service trainings and other trainings required
• Attend case presentations and conferences
• Other duties as assigned

Requirements

• Associate's degree required. Bachelor's degree from accredited four-year College or University in Human Services or related field preferred
• Experience in Human Services field preferred
• Bilingual Spanish candidates preferred
• Must have valid driver's license and insured auto
• Must be able to work flexible hours

This position is eligible for a $2500 retention bonus.
