About the Role

The Case Manager is responsible for the interdisciplinary coordination of care for a designated patient population. This includes reviewing all inpatient admission records to ensure proper utilization of hospital resources and appropriate level of care, assessing discharge needs, coordinating discharge plans, collaborating with the interdisciplinary team to achieve financial and quality outcomes, and maintaining and enhancing payor relationships.

Responsibilities

• Review inpatient admission records for proper resource utilization and appropriate level of care.
• Assess and identify discharge needs and coordinate appropriate discharge plans.
• Collaborate with the interdisciplinary team to achieve desired financial and quality outcomes.
• Maintain and enhance relationships with payors.

Qualifications

• Licensure in the state where the hospital resides (preferred).
• Professional discipline may be occupational therapist, physical therapist, registered nurse, respiratory therapist, speech‑language pathologist, or licensed social worker.
• Current Basic Life Support (BLS) certification required.

Experience

• Three (3) to five (5) years of inpatient experience, preferably in an acute, IRF, or LTACH setting.

Pay

$55 per hour.
