About the role
We have full-time, on-site Case Management Nurse opportunities in New Haven and Bridgeport, CT.
Responsibilities
- Conduct comprehensive assessments
- Develop individualized care plans
- Manage discharge planning to support safe and effective transitions to home or other care settings
- Monitor patient progress
- Facilitate communication among healthcare teams and external providers
- Advocate for patient needs while aligning care with evidence-based practices and organizational policies
- Participate in quality improvement initiatives
- Contribute to patient education regarding care plans, resources, and follow-up services
Qualifications
- Strong clinical foundation in Nursing and Medicine, with the ability to apply evidence-based practice in an acute care environment
- Demonstrated expertise in Case Management, including care coordination, interdisciplinary collaboration, and patient advocacy
- Proficiency in Discharge Planning, with skills in assessing post-acute needs, arranging services, and supporting safe transitions of care
- Experience in Utilization Management, including evaluating appropriateness of services, levels of care, and adherence to regulatory and payer requirements highly valued
- Active Registered Nurse (RN) license in the state of Connecticut; BSN preferred
- Relevant certification in case management (e.g., CCM, ACM) is highly desirable
- Excellent communication, critical thinking, and problem-solving skills, with the ability to work collaboratively in a fast-paced clinical setting
- Familiarity with electronic health records, documentation standards, and hospital-based quality and safety practices
