Case Management Coordinator - Psych Acute

Responsibilities

• Captures and reviews files and documentation in the Department.
• Participates actively in Case Management and Treatment Team meetings.
• Reviews patient charts to assess admission and continuation of treatment criteria; gathers and responds to requests for records from fiscal intermediaries; gathers clinical and fiscal information and communicates account statuses.
• Works independently and uses sound judgment.
• Knowledgeable about Federal, State, and intermediary guidelines related to inpatient, acute care hospitalization, and lower levels of care.
• Coordinates discharge referrals as requested by clinical staff, fiscal intermediaries, patients, and families.

Qualifications

• Associate or Bachelor’s degree in healthcare or related field or equivalent work experience in the related field.
• Acute care work experience preferred.
• Intermediate computer skills including data entry.
• Proficient in written and oral communications.
