Case Management Coordinator Per Diem

About the Role

Gathers all relevant data related to a patient’s treatment status, objectively evaluates it to identify barriers, clarifies realistic goals, and seeks potential alternatives to care. Works with the patient/family, primary care physician, and multi-disciplinary team to develop a treatment plan that enhances patient outcomes. Facilitates communication and collaboration between the patient and health team, and advocates for patient empowerment in their own care.

Requirements

• RN required, BSN preferred
• Two to three years of hospital experience required
• Home Care and/or Utilization Review or Case Management experience desired
