Main Function
Gathers all relevant data related to patient’s treatment status; objectively evaluates it to identifies barriers, clarifies realistic goals & seeks potential alternatives to care.
Works With
- Patient/family
- Primary care physician
- Multidisciplinary team
Treatment Plan Development
Develops a treatment plan that enhances patient outcomes; facilitates communication & collaboration between the patient & health team; advocates patient empowerment in their own care.
Education
RN required, BSN preferred
Experience
- Two to three years hospital experience required
- Home Care and/or Utilization Review of Case Management experience desired
