About the Role

Responsibilities
- Assess patient/client needs and develop individualized care plans.
- Coordinate and integrate support services, including medical, social, and community resources.
- Monitor progress and adjust care plans as necessary to achieve optimal outcomes.
- Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care.
- Advocate for patients/clients to ensure access to necessary services and resources.
