About the role

The Case Management Assistant (CMA) completes various duties to enhance the efficiency of the Case Management Department, as well as support the daily functions of the Case Managers. The CMA assists in securing arrangements for the discharge transition and post-acute services and serves as the liaison between the Case Management Department, payers, and service providers. The Case Management Assistant, under the supervision of the Case Manager, assists in discharge planning assessments.

Responsibilities

• Enhance the efficiency of the Case Management Department
• Support the daily functions of the Case Managers
• Assist in securing arrangements for discharge transition and post-acute services
• Serve as the liaison between the Case Management Department, payers, and service providers
• Assist in discharge planning assessments under the supervision of the Case Manager

Requirements

• Two or more years of work experience in a healthcare environment
• If not LPN, RN, or Social Worker, co-signature required by Case Manager and/or RN for all documentation assistance
