Under the direction of the Social Work Manager in Case Management, the Case Management Assistant (CMA) provides care and service safely and efficiently for a full range of services to patients of all ages and their families. The CMA supports the operation and communications of the department for both point-of-entry and inpatient settings, collaborating with the healthcare team as directed by the Case Manager and Social Worker.

Responsibilities

  • Maintain patient, employee, physician, and corporate confidentiality; respect the rights, privacy, and property of others.
  • Communicate timely, relevant, and accurate information to Case Managers and professionals involved in patient care; maintain routine communication with case managers on referral request status.
  • Communicate Social Work and Case Management referrals to assigned staff and notify the department director/manager when staff cannot facilitate the referral.
  • Receive, screen, and route telephone calls and written communications to appropriate staff; return calls and relay messages as needed; inform staff of urgent correspondence.
  • Communicate with payers to verify patient benefits at the case manager’s request; alert staff to adverse determination responses from insurance companies.
  • Fax surgery procedure updates to Business Office staff for Medicaid reimbursement coordination.
  • Receive and record new or changed contact information for the sitter’s list.
  • Route contact information from Home Health agencies to appropriate staff to facilitate continuity of care.
  • Obtain necessary forms to initiate referrals for care transitions; distribute post-acute placement requests as directed by the RN, CM, or SW.
  • Monitor patient progression toward desired outcomes; facilitate discharge planning, resource referrals, and patient education.
  • Participate in RN, CM, and SW team planning meetings; coordinate resources and services to support team goals and discharge plans.
  • Continuously monitor patients from admission through discharge to ensure timely referrals and coordination of post-acute needs.
  • Ensure data integrity by running reports to validate correct entry into the hospital system.
  • File and maintain insurance communication documents and tracking logs.
  • Maintain and restock social work and case management forms as needed.
  • Complete special projects or assignments to support department operations.
  • Perform the professional, clinical, or technical competencies of the assigned unit or department.

Requirements

  • High School Graduate.
  • 3 or more years of broad administrative or secretarial experience.

Preferred Qualifications

  • Associate’s or Bachelor’s Degree.
  • Healthcare background.

Skills

  • Some knowledge of contractual reimbursement methodology.
  • Some knowledge of community agencies and resources.
  • Excellent verbal and written communication skills.
  • Strong customer service skills.
  • Ability to manage multiple tasks simultaneously.
  • Organizational skills and commitment to confidentiality.
  • Experience with MS Office programs.
