Case Management Assistant

About the role

The Case Management Assistant (CMA) works under the direction of the RN case manager or social worker to assist with facilitating patient discharges requiring post-acute placement or services, and identifies and reports barriers to discharges. The CMA provides administrative support in Case Management functions.

Responsibilities

• Assist with facilitating patient discharges.
• Identify and report barriers to discharges.
• Provide administrative support in case management functions.

Qualifications

• Associate's degree preferred.
• Minimum of one-year experience in a healthcare setting required.
• Medical terminology required.
• Healthcare professional (PCT, CNA, EMT) preferred.
