About the Role
This role supports the Case Management Nurses in various tasks, including monitoring patients on the hospital census, scheduling patient appointments for hospital follow-ups and the High-Risk Team, and facilitating the coordination of authorized Referral Services.
Responsibilities
- Utilizes multiple sources to compose an accurate daily Hospital Census capturing all Secure Horizon / Evercare patients in the hospital, SNF, outpatient surgery facilities.
- Distributes census per protocol to appropriate physician and staff.
- Based on hospital discharges, contacts patients/families to schedule timely Hospital Follow-up appointments.
- Communicates patient concerns promptly to the Case Manager nurses for attention and follow-up.
- Schedules patients to the High-Risk Team of providers for appropriate visit/care based on HRT audit.
- Assists in tracking High Risk patients and completion of audit plan.
- Receives inbound calls to assist patients with health plans or care-related issues.
- Assists patients with information about community resources available to support their healthcare/social needs.
- Arranges transportation for patients to/from healthcare visits.
- Assists with producing letters, faxing documents, and mail outs (certified and other).
- Maintains strictest member and company confidentiality.
- Performs other duties as assigned by supervisor.
Requirements
- All duties performed will be done accurately and in a timely manner.

- Excellent written and verbal communication skills.
- Strong attention to detail and exceptional organizational skills.
- Manual dexterity, ability to sit, stand, stoop, reach, kneel, crouch, bend, and walk for extended periods.
- Ability to lift up to 40 lbs. without assistance.
- Close vision and ability to adjust focus.
- Ability to work efficiently under pressure.
Work Environment
Depending upon the area assigned, may be 100% clinical setting or office setting in a clinic environment. Exposure to communicable diseases, bodily fluids, toxic substances, ionizing radiation, medicinal preparations, and other conditions common to a clinic environment.
