Location: Winter Haven Hospital - Winter Haven, FL. Status: full time.

Responsibilities

• Assists with completing tasks necessary to facilitate patient and family access to agencies, transportation, and services within and beyond acute care
• Responsible for assisting with distributing regulatory documents to identified patients
• Educating patients about the appeal process related to their discharge
• Performing multiple activities that support a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community service
• Requires frequent contact with patients and patient representatives including but not limited to hospital staff, community resource agencies and payor representatives

Requirements

• Education: High School or Equivalent (Preferred Associate's)
• Experience: 1 year in a Medical Related Field OR 1 year of Customer Service skills

Monday - Friday, 8:00 AM - 4:30 PM. Weekend requirement: occasional. On-call: No.
