Part-time position reporting to the Director of Nursing.
About the role
The primary purpose of this position is to provide each assigned resident with routine daily care and services in accordance with the resident’s assessment and care plan, and as directed by supervisors.
Responsibilities
- Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
- Report all changes in residents’ condition to the Health and Wellness Director as soon as practical.
- Record all entries on notes, SPAs, and incident reports in an informative, descriptive manner.
- Report all accidents and incidents observed on the shift to the Executive Director and Health and Wellness Director.
- Agree not to disclose residents’ protected health information and promptly report suspected or known violations.
- Report any known or suspected unauthorized attempt to access the community’s information system.
- Perform only those care tasks for which you have been trained.
- Prepare residents’ rooms (e.g., make beds) and greet/escort residents to their rooms.
- Introduce residents to roommates and other personnel as appropriate.
- Inventory and mark residents’ personal possessions as instructed; assist with packing/unpacking during transfers or discharges.
- Transport residents to new rooms or receiving areas; assist with loading/unloading residents to/from vehicles.
- Maintain an atmosphere of warmth, personal interest, and calm throughout the unit and shift.
- Meet regularly with nursing personnel to identify and correct problem areas or improve services.
- Report all resident complaints and grievances.
- Participate in and receive nursing reports as instructed.
- Follow established policies concerning exposure to blood/body fluids.
- Check residents routinely to ensure personal care needs are met, especially for those unable to call for help.
- Assist residents with meals (e.g., identifying food arrangements, after-meal care).
- Keep residents’ water pitchers clean, filled, and within easy reach.
- Perform daily care functions (e.g., dental/mouth care, bathing, dressing, hair care, nail care, shaving).
- Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
- Assist with lifting, turning, moving, positioning, and transporting residents using proper techniques and equipment.
- Perform restorative and rehabilitative procedures as instructed.
- Assist in preparing residents for medical tests, examinations, and physical therapy.
- Observe and report pressure areas, skin breakdowns, and injuries of unknown source.
- Record and report data as instructed; perform special treatments as directed.
- Immediately notify the Nurse of any resident leaving or missing from the facility.
- Use only equipment you have been trained to operate; report defective equipment.
- Maintain confidentiality of all resident care information, including protected health information.
- Report allegations of resident abuse and/or misappropriation of resident property.
- Honor residents’ refusal of treatment and report such requests to supervisors.
- Provide back rubs, assist with bowel/bladder functions, and maintain intake/output records as instructed.
- Check and report bowel movements and stool character as instructed.
- Utilize protective and adaptive equipment as instructed.
- Provide eye and ear care (e.g., cleaning eyeglasses/hearing aids).
- Weigh and measure residents; record temperatures, pulse, and respirations (TPRs) as instructed.
- Assist with end-of-life care and post-mortem care as instructed.
- Follow work assignments and schedules; cooperate with inter-departmental personnel.
- Notify the facility when late or absent from work.
- Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
Requirements
- High school diploma or equivalent.
- Ability to read, analyze, and interpret professional journals, technical procedures, or governmental regulations.
- Ability to write reports, business correspondence, and procedure manuals.
- Ability to effectively present information and respond to questions from managers and employees.
- Proficiency in applying mathematical concepts (fractions, percentages, ratios, proportions) to practical situations.
- Ability to solve practical problems with limited standardization and interpret instructions in written, oral, diagram, or schedule form.
- Proficiency in Microsoft Suite products.
Physical Demands
- Occasionally lift and/or move up to 25 pounds.
- Prolonged use of desktop or laptop computer.
- Regularly required to sit, stand, walk, talk, read, or hear.
- Frequent use of office-related equipment (copier, scanner, fax, telephone, calculator).
Work Environment
The noise level in the work environment is usually low to moderate.
Pay
$22.00–$23.50 per hour depending on experience and education. Additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
