About the role
On Lok PACE is a non-profit organization dedicated to the care of elders. The PACE (Program of All-Inclusive Care for the Elderly) model aims to address the challenges faced by frail seniors, their families, and caregivers by providing community-based care. Participants receive in-home care services and transportation to On Lok PACE centers for primary medical care, social and recreational activities, and other senior care services.
Responsibilities
- Provides personal care/escort assistance according to the individual care plan, including grooming, dressing, peri-care, showers, bathing, toileting, housekeeping, meal preparation/feeding, and laundry.
- Reports observed changes in the participant's condition to the interdisciplinary team.
- Demonstrates safe practices by utilizing proper body mechanics, adhering to universal precautions, and maintaining environmental safety.
- Interacts and provides services to participants, families, and the interdisciplinary team in a professional manner, encouraging participation in conversation and activities.
- Provides onboarding and ongoing training for other Caregivers using manuals and formal training materials as a guide.
- Actively participates in departmental meetings and family meetings as needed.
- Performs other related duties as assigned.
Requirements
- Minimum one-year experience working with frail or elderly required.
- Understanding of the PACE model gained through personal experience or research.
- Must be flexible and have a positive attitude working across multiple settings.
- Ability to work independently and as part of an interdisciplinary team.
- Knowledge and use of safety equipment for lifting; ability to correct others positively.
Qualifications
- Physical requirements: Ability to stoop, reach, stand, walk, push, pull, lift, grasp, talk, hear, and repeat motions of the wrist and hands. Must have visual acuity.
- Must be able to lift/move weight within individual physical ability and use appropriate lifting devices or request assistance when needed.
- Basic English reading and writing required; bilingual Spanish is required.
- Basic computer skills; ability to use or learn multiple applications such as email and electronic health records.
