Responsibilities

• Provide compassionate one-on-one support, resource navigation, and referral assistance to caregivers through the Caregiver Concierge platform, connecting individuals with local, regional, and statewide services and resources.
• Aid in the development, testing, implementation, and ongoing evaluation of the Caregiver Concierge mobile application, ensuring a positive user experience, accurate resource information, and meaningful data collection.
• Collaborate with the State and organizational leadership to identify priority chronic health conditions and caregiver populations served through the Caregiver Concierge initiative.
• Develop, maintain, and regularly update a comprehensive statewide directory of caregiver resources, healthcare providers, educational opportunities, support groups, community programs, and cancer-related services.
• Maintain and promote a statewide calendar of caregiver support groups, educational classes, workshops, and related events available through community partners.
• Conduct outreach and build collaborative relationships with healthcare systems, nonprofit organizations, community agencies, and other stakeholders to expand caregiver support services and increase statewide access to resources.
• Promote awareness and utilization of the Caregiver Concierge platform through community outreach, presentations, marketing efforts, and partnership development.
• Facilitate virtual evidence-based caregiver education programs, support groups, and educational classes, while conducting occasional in-person workshops, presentations, and outreach events throughout South Dakota.
• Respond to caregiver inquiries and provide individualized guidance, education, problem-solving, and follow-up to address caregiver needs and connect individuals with appropriate services.
• Collect, maintain, analyze, and report program data, participant engagement, outcomes, and resource utilization to support program evaluation, grant reporting, and continuous quality improvement.
• Prepare and submit timely monthly program reports, supporting documentation, and other grant deliverables in accordance with contract requirements.
• Absorb and maintain program expenditures, maintain required documentation, and support grant reimbursement and financial reporting processes.
• Participate in state-directed meetings, programmatic workgroups, webinars, trainings, conferences, and collaborative initiatives to support implementation and continuous improvement of the Caregiver Concierge program.
• Collaborate with the State and community partners on initiatives that strengthen caregiver programming and improve access to caregiver support services throughout South Dakota.
• Represent Active Generations professionally in community meetings, outreach events, conferences, and partnership initiatives while serving as an ambassador for caregiver support services across the state.
• Perform other duties as assigned in support of grant objectives, organizational priorities, and caregiver needs.

Qualifications

• High School Diploma or GED required.
• Bachelor's degree in Social Work, Human Services, Healthcare Administration, Public Health, Health Sciences, or a related field preferred.
• An equivalent combination of education and relevant experience may be considered.
• Experience providing direct support, case management, resource navigation, community outreach, education, caregiver services, healthcare services, or nonprofit program coordination is preferred.
• Experience facilitating classes, support groups, or educational programs and working with diverse populations is beneficial.

Skills

• Knowledge of caregiver challenges, aging services, healthcare systems, community resources, and support programs.
• Ability to build and maintain collaborative relationships with caregivers, healthcare providers, community organizations, and other stakeholders.
• Strong interpersonal, communication, and customer service skills with the ability to provide compassionate and supportive guidance to individuals experiencing stress or caregiving challenges.
• Ability to facilitate educational programs, presentations, and group discussions in both virtual and in-person settings.
• Strong organizational skills with the ability to manage multiple priorities, maintain accurate records, and meet deadlines.
• Ability to collect, track, and report program data accurately.
• Proficiency with Microsoft Office applications, virtual meeting platforms, database systems, and web-based applications.
• Ability to learn and effectively utilize emerging technologies, including caregiver support platforms and digital resource tools.
• Ability to work independently while contributing to a collaborative team environment.

Pay

The salary range for this position is $40,000 - $50,000 annually, depending on experience and qualifications.

Schedule

This position is full-time and requires flexibility to work evenings, weekends, and overnight travel as needed.
