Caregiver (Certified Home Health Aide), ElderONE (Per Diem)

Responsibilities

• Counsels and educates patients and families regarding health maintenance, nutrition, and self-care.
• Assists in the implementation of the patient’s treatment plan under the direction of the RN.
• Provides routine nursing services for patients as directed by the RN.
• Completes required documentation in an accurate and timely manner.
• Attends and participates in department/facility meetings as required.
• Maintains clinical competency by pursuing continuing education opportunities.
• Performs other duties as assigned.

Requirements

• Licensed Practical Nurse (LPN) license in the State of New York.
• Basic Life Support (BLS) Certification from the American Heart Association (AHA).
• Graduate from an accredited School of Practical Nursing.
• Strong communication, customer service, and interpersonal skills are highly desired.
• One year of experience working with a frail or elderly population is preferred.

Qualifications

• Physical Requirements: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
• Travel: Occasional travel to additional locations may be required.
• Mileage Reimbursement: Provided for occasional travel.

Pay

$18.65 - $20.00 per hour.

Schedule

Evening (3:00pm to 11:30pm) and Night (11:00pm to 7:30am) support. 2 weekend shifts per month and one summer and one winter holiday required. Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY.
