About the role

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value-based model.

Responsibilities

• Provide safe delivery of personal care services to participants at the WelbeHealth day center.
• Collaborate with different departments within our day centers, building strong connections not only with participants, but with team members as well.
• Act as a medical escort when needed, assisting participants with boarding and exiting WelbeHealth transportation vehicles.
• Maintain personal care supplies such as incontinence supplies and grooming items.
• Oversee activity on the day center floor to assure participant safety.
• Assist with meal service, ensuring participants receive the correct meals, as well as assist with feeding when needed.

Requirements

• High school diploma or equivalent required.
• Certification of Home Health Aide (HHA) or Certified Nursing Assistant in California (CNA) preferred.
• Minimum of one (1) year of experience providing personal care to seniors.
• Willingness and ability to safely drive to and from participant escort assignment locations, including but not limited to medical and care appointments in the program service area.
• CA Food Handlers Certification within 30 days of hire.
• Obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Qualifications

• Must be able to safely lift and transfer participants.
• Must be able to work flexible hours, including evenings and weekends.

Skills

• Strong interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of basic first aid and CPR.
