About the role

Welcome to WelbeHealth PACE, where we empower seniors to live independently and with dignity. Our mission is to provide comprehensive medical care and community-based services to the most vulnerable seniors, ensuring they receive better quality and compassion in a value-based model.

Responsibilities

• Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes, including bathing, dressing, grooming, dressing, toileting, and physical lifting when transferring participants.
• Assist participants with Instrumental ADLs, such as laundry, light housekeeping, shopping, and light meal preparation.
• Measure and document participants’ vital signs as instructed.
• Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes.
• Provide translation services as needed.

Requirements

• High school diploma or equivalency.
• Current Basic Life Support (BLS)/First Aid Certification.
• Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred.
• Bilingual preferred.
• Registered with Home Care Registry prior to start date.
• Obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred.

Qualifications

• Must be able to lift and move participants as needed.
• Must be able to perform tasks requiring physical effort, including heavy lifting and moving of participants.
• Must be able to communicate effectively with participants and their families.
• Must be able to work flexible hours, including evenings and weekends, as needed.

Skills

• Strong interpersonal skills.
• Ability to work independently and as part of a team.
• Knowledge of basic medical terminology and procedures.
• Ability to follow written and verbal instructions.

Pay

Base pay details will be provided during the interview process.

Schedule

The schedule will be determined based on participant needs and availability, typically consisting of visits to 2-5 participants per day, with shorter individual shifts that still add up to an 8-hour workday.

We have a vaccination policy for all our employees. Proof of vaccination, or a vaccine declination form will be required prior to employment. We maintain required infection control and PPE standards and have requirements relevant to all team members regarding vaccinations.
