About the role

Self-direction is one-on-one caregiving in the client’s home for members who act as their own employer. Community Horizons helps members find care so they can remain in their own homes.

Responsibilities

• Assist with daily personal cares and hygiene needs
• Provide support with dressing, grooming, toileting, showers, and other routine care tasks
• Offer companionship and engage in meaningful interaction and activities
• Accompany client on outings and community activities
• Spend time outdoors, as the client enjoys nature and being outside
• Help maintain a calm, positive, and safe environment
• Support mobility and daily routines as needed
• Light housekeeping related to client care
• Monitor comfort, mood, and overall wellbeing
• Help move clients in and out of beds, baths, wheelchairs, or automobiles and assist with dressing and grooming
• Bathe clients
• Change bed linens, wash and iron laundry, and clean or assist with personal care
• Entertain, converse with, or read aloud to clients to keep them mentally healthy and alert
• Perform a variety of duties as requested by the client, such as obtaining household supplies or running errands
• Administer prescribed oral medications under written direction of a physician or as directed by a home care nurse or aide, and ensure clients take their medicine

Qualifications

• Patient, kind, and dependable
• Comfortable working with nonverbal individuals with some behaviors
• Enjoys outdoor activities and being active in the community
• Observant and able to communicate through nonverbal cues and routines
• Respectful, open-minded, and compassionate
• Previous caregiving or DSP experience preferred
• Valid driver’s license preferred for outings and appointments
