About the role

The primary purpose of your job position is to provide each assigned resident with routine daily care and services in accordance with the resident’s assessment and care plan, and as directed by your supervisors.

Responsibilities

  • Use PCC to ensure correct resident information when assisting with ALDS, meals, etc.
  • Report all changes in residents’ condition to the Health and Wellness Director as soon as practical.
  • Record all entries on notes, SPAs, and incident reports in an informative, descriptive manner.
  • Report all accidents and incidents observed on the shift to the Executive Director and Health and Wellness Director.
  • Agree not to disclose residents’ protected health information and promptly report suspected or known violations.
  • Report any known or suspected unauthorized attempt to access the community’s information system.
  • Perform only those care tasks for which you have been trained.
  • Ensure the resident’s room is ready (bed made, personal possessions inventoried and marked).
  • Greet residents, escort them to their room, and introduce them to roommates and personnel.
  • Assist residents with packing/unpacking personal possessions during room transfers or discharges.
  • Transport residents to new rooms or the receiving area; assist with loading/unloading residents to/from vehicles.
  • Maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
  • Meet regularly with nursing personnel to identify and correct problem areas or improve services.
  • Report all resident complaints and grievances.
  • Participate in and receive the nursing report as instructed.
  • Follow established policies concerning exposure to blood/body fluids.
  • Make and prepare beds; add extra covers as requested.
  • Check residents unable to call for help frequently; answer resident calls promptly.
  • Assist residents with meals (identify food arrangements, perform after-meal care).
  • Keep water pitchers clean, filled, and within easy reach.
  • Perform all assigned tasks in accordance with established policies and procedures.
  • Cooperate with inter-departmental personnel to maintain adequate services.
  • Notify the facility when late or absent from work.
  • Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals.
  • Assist residents with daily functions (dental/mouth care, bathing, dressing/undressing, hair care, nail care).
  • Keep residents dry by changing gowns, clothing, and linens when wet or soiled.
  • Assist with lifting, turning, moving, positioning, and transporting residents.
  • Perform restorative and rehabilitative procedures as instructed.
  • Observe and report pressure areas, skin breakdowns, and injuries of unknown source.
  • Record and report data as instructed; perform special treatments as directed.
  • Immediately notify the Nurse of any resident leaving or missing from the facility.
  • Use only equipment you have been trained to use; report defective equipment.
  • Maintain confidentiality of all resident care information; report unauthorized disclosures.
  • Report all allegations of resident abuse and/or misappropriation of resident property.
  • Honor residents’ refusal of treatment and report such requests to your supervisor.
  • Assist with bowel and bladder functions; maintain intake and output records as instructed.
  • Provide eye and ear care (cleaning glasses/hearing aids, installing hearing aids).
  • Weigh and measure residents; measure and record temperatures, pulse, and respirations (TPRs).
  • Assist with the care of end-of-life residents and post-mortem care as instructed.

Requirements

  • High school education or equivalent.
  • Ability to read, analyze, and interpret professional journals, technical procedures, or regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions.
  • Ability to apply mathematical concepts (fractions, percentages, ratios) to practical situations.
  • Ability to solve practical problems with limited standardization.
  • Ability to interpret instructions in written, oral, diagram, or schedule form.
  • Proficiency in Microsoft Suite products.

Physical Demands

  • Occasionally lift and/or move up to 25 pounds.
  • Prolonged use of a desktop or laptop computer.
  • Regularly required to sit, stand, walk, talk, read, or hear.
  • Frequent use of office-related equipment (copier, scanner, fax, telephone, calculator).

Work Environment

  • Low to moderate noise level.

Pay

$17.00 – $18.50/hour, commensurate with experience.
