CareConnect Registered Nurse (RN) – Part-time - In home Urgent Care - Youngtown, Ohio

We are seeking a highly skilled, compassionate Registered Nurse (RN) to deliver high-quality, patient-centered care through a combination of in-home patient visits, telehealth consults with Nurse Practitioners or Physicians, and administrative tasks. This role supports urgent/episodic interventions, working in partnership with physicians, nurse practitioners, and interdisciplinary care teams to address patient needs across the care continuum. The RN will operate with a patient-first mindset, balancing clinical judgment with care coordination, education, and documentation.

About the Role

The CareConnect Team at WellBe supports timely response to patients’ changing needs. This role primarily involves delivering urgent, in-home care, conducting home safety evaluations, and collaborating with interdisciplinary teams to develop and implement individualized patient care plans. While there is no direct supervisory responsibility, the RN will function as part of an integrated care team and foster positive working relationships with leadership, management, and community partners.

Daily local travel to meet patients and the community is essential. The role requires adherence to state and federal regulatory standards and active participation in training and professional development.

Responsibilities

• Deliver urgent, in-home patient care under the direction of a licensed provider, including initial assessments, routine follow-up visits, and ongoing care coordination.
• Conduct comprehensive home safety evaluations and document findings during patient visits.
• Perform clinical interventions as directed by the overseeing provider, including:
  • Medication administration (PO, SL, IM, IV)
  • Wound care
  • Point-of-care testing
  • Phlebotomy
  • Foley catheter care
  • IV fluid administration
  • Other approved nursing procedures
• Obtain patient histories, review medications and treatment plans, and perform physical assessments.
• Identify and address health concerns observed or reported during home visits.
• Maintain accurate, timely, and complete patient records, ensuring documentation is updated at each patient encounter.
• Provide patient and caregiver education as directed by the APP-Nurse Practitioner and interdisciplinary care team.
• Support care transitions, end-of-life discussions, palliative care interventions, and closure of preventive care and HEDIS quality gaps.
• Complete and submit all required documentation, reports, and administrative paperwork within established timelines.
• Participate in Interdisciplinary Team (IDT) meetings, providing updates and insights from patient and family interactions.
• Collaborate closely with APPs, nurses, social workers, behavioral health specialists, and other care team members to develop and implement individualized patient care plans.
• Attend and actively participate in required training programs, including medication administration, clinical equipment use, new protocols, and ongoing professional development.
• Build and maintain positive relationships with local healthcare providers, community organizations, caregivers, and referral sources.
• Utilize internal and external resources to address patient needs, including behavioral health services and community-based programs.
• Maintain all required active, unrestricted state licensure and certifications.
• Adhere to professional standards of practice, ethical conduct, patient confidentiality requirements, and organizational policies.
• Perform additional duties and responsibilities as assigned to support organizational goals and patient care excellence.

Requirements

• Active State RN licensure required; compact licensure preferred.
• 3+ years as an RN in a direct, patient-facing care delivery role (required).
• CPR Certification is required.
• Prior telehealth experience (preferred).
• Home care experience (preferred).
• Previous experience in hospice, palliative care, or geriatrics strongly preferred; other complex patient healthcare experience will be considered.
• Valid driver’s license with an acceptable driving record.
• Proof of active auto insurance policy.

Skills

• Ability to conduct in-home visits and use telehealth platforms.
• Proficiency with Microsoft Office (Excel, Word), G-suite, Microsoft 365, Electronic Health Records, scheduling software, and Ring Central applications.
• Excellent verbal, written, presentation, and interpersonal communication skills.
• Strong communication skills for interacting with patients, families, and remote healthcare professionals.

Pay

$37.25 - $54.12 per hour, plus mileage and full benefits.

Schedule

Tentative working hours: 10 AM – 7 PM EST.

Work Conditions

• Ability to lift up to 20 lbs; occasional lifting or transferring of patients may involve up to 50 lbs or assistance with weights exceeding 100 lbs.
• Ability to stand for extended periods.
• Daily local travel to patient locations (e.g., home, hospital, SNF) or community resource locations.
• Fine motor skills and visual acuity required.
