Care Transitions Coordinator Home Health RN LVN
Responsibilities
Affords patients the opportunity to navigate post-acute care processes, aiming to enhance patient outcomes and improve referral source satisfaction.
Utilizes evidence-based clinical guidelines, preventative strategies, protocols, and metrics in developing transition plans that prioritize patient-centered care, ensuring quality and efficiency in post-acute care delivery.
Engages in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, physician groups, and executive-level entities, representing the branch's interests effectively.
Requirements
- Must be a graduate of an approved nursing, therapy, or social work program.
- Must be licensed in the state of practice.
- Must have at least two years of relevant field experience.
- Must demonstrate knowledge of federal, state, and local regulations governing Medicare-certified home health and hospice operations.
- Must possess basic technology skills, including the ability to operate a mobile device.
Qualifications
- Education and Experience, Essential:
- Graduate from an approved nursing, therapy, or social work program.
- Licensed in the state of practice.
- Two years of demonstrated field experience.
- Experience and understanding of federal, state, and local laws and regulatory guidelines governing Medicare-certified home health and hospice operations.
- Demonstrated basic technology skills, including operation of a mobile device.
Qualifications, Preferred
- Registered Nurse or Physical Therapist preferred.
- Three years of field experience preferred.
- Previous experience in home health or healthcare sales preferred.
Requirements
- Must hold a valid state driver license.
- Maintain automobile liability insurance as required by law.
- Maintain reliable transportation in good working condition.
- Capable of driving safely in various weather conditions.
