Care Transitions Coordinator Home Health

Responsibilities

• Affords patients with guidance through post-acute care processes.
• Utilizes evidence-based clinical guidelines, preventative strategies, protocols, and metrics in developing transition plans tailored to individual patient needs.
• Engages in strategic partnerships with health systems, hospitals, inpatient facilities, physicians, and physician groups.

Qualifications

• Graduate of an approved nursing, therapy, or social work program.
• Licensed in the state of practice.
• At least two years of relevant field experience.
• Comprehension of federal, state, and local regulations governing Medicare-certified home health and hospice operations.
• Basic proficiency with technology, including operation of a mobile device.

Requirements

• Valid state driver license.
• Automobile liability insurance as required by law.
• Dependable transportation in good working condition.
• Safety in operating an automobile in various weather conditions.
