Care Transition Supervisor
About the role
The Care Transitions Supervisor provides operational supervision and day-to-day oversight of the Care Transitions team within the Virtual Medical Team (VMT). This position is responsible for coordinating post-acute care transition services, supervising Care Transitions Liaisons and support staff, and ensuring timely, patient-centered coordination of care for patients transitioning from hospitals, skilled nursing facilities (SNFs), emergency departments, and other care settings.
This is a hybrid role in Berkeley. This is a full time, exempt, benefit eligible position. LifeLong Medical Care is a multi-site, Federally Qualified Health Center (FQHC) with a rich history of providing innovative healthcare and social services to a wonderfully diverse patient community. Essential Functions
- Provides day-to-day operational oversight of the Care Transitions team within the Virtual Medical Team (VMT), ensuring efficient workflows and high-quality patient care.
- Supervises, recruits, hires, trains, coaches, develops, and evaluates Care Transitions Liaisons and assigned staff, fostering a collaborative, patient-centered culture of continuous improvement.
- Manages staffing, workload distribution, scheduling, and operational priorities to ensure appropriate coverage and timely patient outreach.
- Oversees care transition workflows for patients discharged from hospitals, emergency departments, skilled nursing facilities, and other care settings, ensuring timely follow-up, care coordination, and adherence to organizational standards.
- Collaborates with Care Transitions Nurses, providers, Primary Care, Population Health, Referral Management, Behavioral Health, and interdisciplinary teams to address barriers to care and improve patient outcomes.
- Serves as the operational liaison between the Virtual Medical Team, clinic leadership, hospitals, skilled nursing facilities, health plans, and community partners to support seamless transitions of care.
- Monitors operational performance, patient work queues, quality metrics, and documentation compliance, implementing process improvements that reduce readmissions, improve patient experience, and support organizational goals.
- Serves as the escalation point for complex care transition issues requiring cross-departmental coordination and problem resolution.
Administration, Compliance & Budget
- Ensures compliance with LifeLong policies and applicable federal, state, and local regulations.
- Maintains operational documentation and reporting requirements related to Care Transitions activities.
- Supports auditing of documentation and workflow compliance within the electronic health record.
- Assists with policy and procedure development and updates.
- Participates in departmental planning, resource allocation, and operational improvement initiatives.
- Supports budget stewardship by effectively managing staffing resources and operational efficiencies.
Community Relations And Coordination
- Functions as a liaison to other community organizations.
- Actively engages program volunteers, committee members, board members, partnering organizations, and funders to build community support.
- Assists with marketing and outreach efforts, including community presentations and educational events.
- Other duties as assigned.

Pay
$70,400 - $75,000/year
