Job Description
The Care Transitions Navigator will coordinate activities that promote quality outcomes, patient throughput and discharge planning while supporting a balance of optimal care and appropriate resource utilization. The Care Transitions Navigator will identify potential barriers to patient throughput and quality outcomes minimizing delays in discharge plans.
Job Requirements
- Education: Bachelor's degree in Social Work required, Master's degree in Social Work preferred
- Licenses and/or Certifications: LMSW/LBSW, or RN as licensed by the Texas Board of Examiners, CCM or ACM preferred
- Work Experience: 1 year of experience in health related setting, Hospital case management experience preferred
- Related Work Experience and Other Skills: Ability to work independently, effective time management, problem-solving skills, detail-oriented with strong organizational skills, ability to multi-task, ability to react calmly, objectively, and effectively in emergency situations, maintain a high standard of ethical behavior and personal expertise in the performance of your work.
