Sentara Health is seeking to hire a qualified individual to join our team as a Care Transition Coordinator. This is a full-time day shift float position that will cover locations in the Hampton Roads area where CTC is out of the office for reasons such as PTO or call out.
Responsibilities
- Responsible for the identification and assessment of home care and hospice needs of patient population
- Coordination of these services to ensure a smooth transition to the next site of care
Qualifications
- Licensed RN, LPN, RT or BSW required
- Proof of education as LPN, RN, RT or BSW required
- 1 year of home care, discharge planning or case management experience required
