Job Description Position Summary We are seeking a compassionate, organized, and culturally responsive Care Coordinator / Community Resource Specialist to support clients throughout their treatment journey. This position serves as a vital link between clients, families, healthcare providers, and community resources, ensuring individuals receive the services and support necessary to achieve positive outcomes. The successful candidate will engage clients from referral through treatment, coordinate care and services, assist with benefits and insurance enrollment, and provide resource navigation and advocacy. This role requires strong relationship-building skills, attention to detail, and a commitment to serving diverse populations. Key Responsibilities Engage clients from the point of referral through successful completion of treatment services.Coordinate care and support services to address client needs and treatment goals.Assist clients with benefits eligibility determination and enrollment in public and private insurance programs.Conduct social needs screenings and identify barriers to care.Provide resource referrals and service navigation assistance.Support clients and families in accessing clinical, behavioral health, and community-based services.Develop and maintain strong relationships with community agencies, referral sources, and service providers.Facilitate community resource linkage and coordinate appropriate referrals.Maintain accurate client records, service data, and required reports.Ensure compliance with HIPAA regulations, Joint Commission standards, agency policies, and documentation requirements.Participate in interdisciplinary team meetings and collaborative care planning.Contribute to quality improvement initiatives and program development efforts.Provide ongoing support, education, and advocacy for clients and their families.Perform other duties as assigned. Required Experience Education Bachelor's degree in Social Work, Human Services, or a related field preferred.Candidates with an Associate's degree and relevant human services experience will be considered. Experience Minimum of three (3) years of direct experience in Substance Use Disorder (SUD) treatment.Knowledge of case management protocols and evidence-based practices.Experience working with diverse and underserved populations.Demonstrated cultural competence and commitment to equitable service delivery.Strong verbal and written communication skills with the ability to effectively engage clients, families, healthcare providers, and community partners.Ability to work independently while managing multiple priorities.Experience maintaining accurate records and documentation.Proficiency in Microsoft Office applications, including Word and Excel, and general computer skills in a Windows environment. Required Fluency in Spanish and English (spoken and written).
