Care Navigator - BHN

Summary

Aid unit leadership in assessing patient functioning and developing/implementing treatment plans based on patient needs.

Responsibilities

• Provide direct care services to patients individually and in groups, focusing on activities of daily living, social, and psychiatric rehabilitation as per the treatment plan.
• Develop and implement comprehensive treatment plans, incorporating patient needs and preferences as identified through surveys, collaterals, and the treatment team.
• Document and communicate patient responses to treatment effectively and promptly, adhering to program policies and procedures.
• Intercede on behalf of patients to ensure they have access to necessary services.
• Participate in the peer review process and utilize quality improvement techniques to enhance service quality and reduce costs.
• Engage in multidisciplinary team meetings, offering feedback, maintaining professional relationships, and providing program-specific recommendations.
• Pursue professional growth and development through participation in educational programs, literature reviews, in-service meetings, and workshops.

Requirements

• Associate's degree (AS) is required.

Physical Requirements

• Sedentary work, involving sitting most of the time with occasional walking or standing.

Licenses / Certifications

• S - Sedentary Work - Exerting up to 10 pounds of force occasionally.

Education

• Associate's degree (AS) is required.

Pay Range

• $20.00 - $26.00 per hour.
