Care Manager- RN - Inpatient Unit- Columbia Day Shift – Weekends

About the role

Case Managers at NewYork-Presbyterian/Columbia University Medical Center are guardians of continuity of care, leading the full spectrum of patient care from pre-admission to discharge. They ensure quality outcomes and serve as a strong liaison between the interdisciplinary team.

Responsibilities

• Provide intensive care management for adult patients with complex medical needs
• Serve as a liaison between the interdisciplinary team of social workers, physicians, physical therapists, occupational therapists, cardiologists, and intensivists
• Develop individualized care plans to ensure optimal length of stay
• Prevent hospital readmissions and set outpatient service needs
• Ensure continuity of care from admission to discharge

Requirements

• 2 years of case management/care coordination experience in an inpatient, HMO, insurance, or other outpatient setting
• 1 year of leadership experience in a healthcare setting
• Master's Degree in healthcare related field
• Fluency in Spanish

Preferred Criteria

• Bachelor's degree in Nursing (BSN)
• Certified Registered Nurse (RN) licensure
• 3 to 5 years of health care experience in an acute care setting
• Superior communication, problem-solving skills, business acumen, and interpersonal skills

Pay

$134,900 - $168,000/Annual

Schedule

Full time day shift with holiday coverage as needed
