Care Manager RN Full-time Variable
About the role
The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient-centric care management services for at-risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/or review, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy.

Requirements
- Associate's Degree in Nursing degree/diploma upon hire
- California Registered Nurse License upon hire
- California Fire and Life Safety Card - National Organization within 30 days of hire
- 2 years Acute care experience in a Medical Surgical or Inpatient setting
- 1 year experience in care management or utilization review in any setting
Qualifications
- Bachelor’s Degree in Nursing or higher within 3 years of hire
- Case management certification (ACM or CCM)

Pay
The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $57.28 - $88.92.
Schedule
This position is Full-time and will work 8-hour, Variable shifts.
