Care Manager RN - Float Personnel

Role Overview

The Team Based Care (TBC) Manager RN independently utilizes the nursing process to provide safe, therapeutic, and efficient patient care. This role involves conducting shared visits with providers and independent RN visits following standardized protocols and documented workflows. The RN collaborates with team members to coordinate and implement care plans, demonstrating initiative within a Team Based Care Model and embodying core values.

Qualifications & Skills

• Graduate from an accredited nursing school with a California Registered Nurse License
• Minimum of 2 years professional nursing experience
• Preferred: Bachelor's degree in nursing and 1 year of ambulatory care nursing experience

• Provide patient-centered care through shared and independent visits
• Coordinate and execute care plans collaboratively with team members
