Care Manager RN - Emergency Department

About the role

The nurse case manager coordinates the care and service of selected patient populations through the acute care episode, across the continuum. Works collaboratively with inter-disciplinary teams, both internal and external to the organization, to improve patient care through effective utilization and monitoring of healthcare resources and assumes a leadership role to achieve desired clinical, financial, and resource outcomes.

Qualifications

• Bachelor’s degree from an accredited nursing program; applicants without a BSN must actively pursue and attain a BSN within three years of hire (continued employment beyond three years contingent on attaining the BSN).
• Current Washington Registered Nurse License.
• Minimum three years of registered nursing experience in an acute care hospital.
• Preferred: one year of hospital case management experience.
