About the role
Provides care management and coordination support by collaborating with multidisciplinary teams to deliver integrated, quality, and cost-effective member care across various settings.
Responsibilities
- Conducts comprehensive member assessments
- Develops and implements personalized care plans
- Monitors progress
- Facilitates interdisciplinary meetings
- Promotes service integration
- Uses motivational interviewing to support behavior change
- Assists members in overcoming barriers
- Offers resources and guidance
- May handle complex cases involving medication management
- Maintains an active caseload through regular outreach and documentation
Qualifications
- At least 2 years of healthcare experience
- RN licensure
- Understanding of EMR and HIPAA
- Valid driver’s license and reliable transportation
Preferred qualifications: Certification as Case Manager (CCM), Medicare case management, discharge planning, and home health experience are advantageous.
