Care Manager (Registered Nurse/RN)

The RN Care Manager is responsible for coordinating patient care and services, promoting quality through collaboration with patients, families, care teams, payers, and external entities. They ensure safe transitions from the facility to other care settings, adhering to all laws and standards. Qualifications Include Nursing diploma (associate’s or bachelor’s degree) Active RN license in the applicable state Preferred: 1+ year of clinical experience in post-acute care, case management, utilization review, or discharge planning Preferred: CCM or RN BC certification Key Skills And Duties Clear, organized written communication Strong organizational and time-management skills Knowledge of post-acute care laws, regulations, and procedures Ability to make independent decisions and interpret policies Maintain professional relationships and confidentiality Use computer systems effectively Serve as liaison with external resources, train patients and staff, and promote ethical care practices Demonstrate excellent customer service, dependable attendance, and teamwork Physical ability to perform job functions including mobility tasks An emphasis on professionalism, integrity, and compliance with company policies is essential.
