Care Manager (Registered Nurse/RN)

About the role

The RN Care Manager coordinates patient care and services, promoting quality care through collaboration with patients, families, care teams, payers, and external entities.

Responsibilities

• Facilitate safe transitions from the facility to other care settings in compliance with laws and standards.
• Develop clear, organized written communication and manage time efficiently.
• Serve as a liaison with external case managers, physicians, families, and community resources.
• Train and educate patients, families, and care providers.
• Implement care management standards, ethical practices, and departmental policies.
• Ensure patient rights, safety, and confidentiality are prioritized.
• Utilize computer systems for data entry and retrieval.
• Exhibit professionalism, integrity, and adherence to organizational policies and codes of conduct.
• Support organizational goals and demonstrate strong organizational and communication skills.

Qualifications

• Diploma or degree in nursing.
• Active RN license in good standing.

Preferred experience

• Clinical experience in post-acute care, case management, utilization review, or discharge planning.
• Case management certifications such as CCM or RN-BC.

Skills

• Excellent organizational, communication, and decision-making skills.
• Knowledge of regulatory and procedural standards in post-acute care.
• Ability to work independently and as part of an interdisciplinary team.
• Strong customer service orientation with a positive attitude.
• Physical ability to perform essential functions, including mobility and communication skills.
