Formulates care plans

• Collaborates with patient, family, and physician

Knowledge, Skills, And Abilities

• Functions with a high degree of independence
• Ability to delegate tasks
• Strong computer and technology skills
• A working knowledge of community resources
• Knowledge of Home Care Regulations and Third-Party Reimbursement
• Current IV Therapy skills

Education

• Associate's of Nursing [Required]
• Bachelor's of Nursing [Preferred]

Work Experience

• 1+ relevant clinical nursing experience [Required]
• Relevant experience in a Medicare-certified home health agency as a case-manager [Preferred]

Licenses And Certifications

• Registered Nurse (RN) [Required]
• Certified for Oasis Specialist-Clinical (COSC) [Preferred]

• $31.53 - $52.24
