Care Manager Registered Nurse

About the Role

As a Care Manager Registered Nurse, you will provide case management services for inpatient and observation patients, assessing psychosocial, medical, and discharge needs. You will act as a liaison between physicians, patients, families, and outside agencies to ensure smooth care transitions.

Responsibilities

• Identify at-risk patients and coordinate care plans with the healthcare team.
• Collaborate with Social Services for post-acute care arrangements.
• Evaluate care progression using evidence-based tools like InterQual.

Requirements

• Minimum 2+ years in Case Management or 3+ years in a clinical setting.
• Associates Degree in Nursing (Bachelor’s or Master’s preferred).
• Current RN license and BLS certification.
• Experience with InterQual is a plus.

Skills

• Strong communication and collaboration skills.
• Ability to assess and coordinate complex care plans.
• Knowledge of discharge planning and care transition processes.
